    Registered Nurse - Certified Case Manager - Travis Air Force Base!

     

    New opportunity for an experienced, certified Case Manager at the David Grant Memorial Military Medical Center, Travis Air Force Base, Fairfield, CA!

     

    Typical Schedule:

    • Full time, 40 hours/week
    • 0630 - 1730
    • Monday-Friday

    Location Services to include but not limited:

    • Primary Care, Women's Health, Mental Health
    • Pharmacy, Labs, x-ray
    • Same Say Surgery, Physical & Mental Health Therapy

    Responsibilities

    • Telephonic & direct patient interaction
    • Local travel to secure Military Barracks locations only
    • Soft hand-offs from Ambulatory & Inpatient settings

    Qualifications

     

    • Associates or Bachelor's Degree of Nursing from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE)
    • Certification in Case Management OR 30 hours of completed CEU's towards Case Management.
    • Minimum of six (6) years’ experience with at least three (3) years in Case Management. One year of experience must have occurred within the last 24 months. 
    • Full time employment in a nursing field for at least 12 of the last 18 months is mandatory.
    • Basic Life Support (BLS) certification from American Heart Association
