    Social Worker - Case Manager

    Are you an experienced  Social Worker - Case Manager? Would you like an opportunity to give back to our men and women in uniform?

    We are currently accepting resumes for Social Worker - Case Manager to provide medical services at the Department of Defense Military Health System (MHS),

    (Langley AFB) Hampton, Virginia area in anticipation of an upcoming government contract award.
     
    Monday – Friday. Eight (8) hours of duty from 0730 to 1630
    No Federal Holidays
    No weekends

    - Knowledge and ability to effectively apply the following core case management Functions: a) Assessment: Identification of patients for case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; and d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.

    Responsibilities

    Familiarity with Department of Defense (DoD), Federal, State, non-profit Health-care systems and organizations, to include DoD Tri-Service Military Health-care Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is desired.

    Qualifications

     

    • Must hold a Master of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE). 
    • Maintain a current, unrestricted clinical license to independently practice social work in any one of the 50 states, the District of Columbia, Puerto Rico, or the U.S. Virgin Islands. 
    • Have a minimum of two (2) years within the past five (5) years in the independent practice of clinical social work in a mental health setting as a (LCSW)
    • Experience working in a Government setting such a DOD or Department of Veterans Affairs (VA) MTF is preferable.
