    Clinical Pharmacists


     We are currently accepting resume in anticipation of an upcoming government contract award for a Clinical Pharmacists for NH Jacksonville FL and may also support other Clinics affiliated.


    The Pharmacists may be required to provide temporary services at Naval Branch Health Clinic (NBHC) Kings Bay, GA, or NBHC Mayport, FL, as an alternate worksite. All sites are within a 50-mile radius

    Outpatient


    Sunday through Saturday, between the hours of 0600 and 2200


    Shifts and weekend services may be rotated with other Pharmacists, and it is anticipated shall work every other weekend and provide services for no more than 5 Federal holidays

    This is a full time (40 hours) clinic position 

    Responsibilities

    Be responsible for the quality and timeliness of records, reports, and documentation of services provided.


    Review provider written orders for appropriate therapy based on diagnosis, age, kinetic calculations, and any protocols established. Have knowledge of pharmacokinetic dosing as it pertains to complex medication regimens including but not limited to vancomycin, aminoglycosides, theophylline, digoxin, phenytoin and warfarin. Develop appropriate treatment plans and monitor results assessing the patient's outcome to the suggested therapy.


    Meet with patients to obtain a complete medication history, monitor the patient's response to drug therapy, educate and counsel the patient regarding his/her drug therapy. Counsel patients with special emphasis on therapeutic expectations, potential side effects, and any special education required for thorough understanding of the disease state.


     

    Qualifications

    You must have:


     




    • Possess:




    (a) at least a 1 year pharmacy residency or fellowship from an accredited program or


    (b) documented experience of at least 12 months serving in a clinical pharmacist position performing a combination of the following within the preceding 24 months: Anticoagulation, hyperlipidemia, diabetes, or asthma therapy; physical assessments; physician rounding; pharmacokinetics; Board Certified Pharmacotherapy Specialist; P&T Committee membership; or drug/medication usage evaluation. The applicant’s experience shall be documented in letters of recommendation. Not withstanding the aforementioned experience requirements, must have pertinent clinical experience within the past 2 years sufficient to demonstrate current clinical competency for the setting and procedures required by this contract.
