     We are currently accepting resume in anticipation of an upcoming government contract award for a Clinical Pharmacists for NH Jacksonville FL and may also support other Clinics affiliated.


     


     


    NH Jacksonville FL


    Outpatient


     Monday through Friday, between the hours of 0700 and 1800

    This is a full time (40 hours) clinic position 

    Qualifications

    You must have:


     




    • Possess one of the following:


       At least a one year pharmacy residency or fellowship from an accredited pharmacy residency or fellowship program.


      OR


       Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist in a primary care and/or ambulatory care clinic, providing direct patient care services in a combination of the following: anticoagulation, dyslipidemia, diabetes, hypertension, asthma/COPD, heart failure or obesity.


      OR


       Specialty Board Certification (Ambulatory Care, Cardiology, Geriatrics or Pharmacotherapy) by the Board of Pharmacy Specialties.



    • Possess either:
       A Baccalaureate Degree in Pharmacy from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE);
      OR
       Possess a Doctorate degree in Pharmacy (Pharm.D.) from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE).

    • Possess and maintain a current, valid, unrestricted license to practice Pharmacy in any one (1) of the fifty (50) states, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
