    Responsibilities

    Tasks and Duties include but are not limited to:

    • Treat patients with common acute conditions, illnesses or minor trauma within accepted protocols in collaboration with MHP team members (other providers, Nurses, Pharmacists, etc.) and specialists as clinically indicated
    • Conduct post and pre-deployment screenings
    • Engages in proactive population based health management, providing regular surveillance to patient’s need for preventive services and management of chronic disease leveraging information technology decision support tools and the members of the medical home port team
    • Perform and record a health appraisal including physical assessment and evaluation
    • Request diagnostic testing, including but not limited to X-Rays and laboratory tests as deemed necessary

    Qualifications

    Job Requirements:

    • Possess an active and unrestricted Advanced Practice-Nurse Practitioner license
    • Valid unrestricted DEA number
    • Board certification as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners                       
    • Possess a Master’s, Post Master’s, or Doctorate degree from an advanced practice nursing program accredited by the NLNAC or CCNE
    • Possess at least 6 months of full time experience as a Family Nurse Practitioner within the last 24 months working in an Primary Care setting
    • Possess a BLS certification from an American Heart Association program
    • Ability to successfully pass a Government background investigation
